docs: add agent execution rule and update cli framework reference

Document that parallel agent execution models prescribed in specs must
be followed as hard requirements, not collapsed into a single agent
without user confirmation. Also update the architecture description to
reference github.com/spf13/cobra now that the urfave/cli migration is
complete.
