feat: add cardinality limits to metrics registry (#47)

Add MaxCardinality config (default 1000) to prevent memory exhaustion
from unbounded label values. When limit is exceeded, oldest entries
are evicted using FIFO policy. Applies to counters, gauges, and
histograms.
