Document namespace consistency requirement for CRD chart (#4022)

* Add Helm release-namespace annotation to CRD templates (#4005)

When upgrading the toolhive-operator-crds chart with a different namespace
than the original install, Helm rejects the upgrade due to mismatched
ownership metadata. Add an explicit meta.helm.sh/release-namespace
annotation set to toolhive-system on all CRD templates to prevent this
namespace mismatch error.

* Replace hardcoded annotation with namespace validation

Helm overrides meta.helm.sh/release-namespace regardless of template
values, so hardcoding it doesn't work. Instead, add a namespace
validation template that fails helm install/upgrade if the target
namespace is not toolhive-system, preventing the mismatch at the source.

Helm stamps CRDs with a meta.helm.sh/release-namespace annotation at
install time that cannot be changed by subsequent upgrades. Add
documentation explaining that users must install the CRD chart in the
same namespace consistently, and provide a migration command for users
who need to switch namespaces.
