Issue/parser sanitization gaps (#20)

* Add missing sanitization chains to InlineImage, CodeInline, LinkReference, ImageReference

InlineImage now sanitizes CodeBlock and CodeInline before matching.
CodeInline now sanitizes CodeBlock before matching. LinkReference
and ImageReference now sanitize CodeInline in addition to CodeBlock.
Unskips 4 spec tests and updates the CodeInline-inside-CodeBlock
test to assert no false positive match.

* Update issue and parser spec with sanitization chain documentation

Checks off all acceptance criteria. Replaces the outdated
Section._extract input table with a per-extractor sanitization
chain table documenting what each extractor sanitizes internally.
Documents known gaps and design decisions.

* Add table-internal-sanitization issue

Tracks making Table.extract() self-contained by adding internal
CodeBlock sanitization, consistent with all other extractors.

* Fix stale test comments and add spec reminder to table issue

Updates test_reference.py section comment and test_image.py docstring
to reflect the new sanitization chains. Adds explicit acceptance
criterion to table-internal-sanitization issue for updating the
Section._extract() Orchestration spec section.

* Mark parser-sanitization-gaps as completed

* Fix spec contradiction about Table sanitization being self-contained

The spec claimed all extractors sanitize internally, but the table
and orchestration sections note Table relies on external sanitization.
Reworded to call out the Table exception explicitly.
