refactor: Address PR review — cache version, standardize commit hash

- Cache version/commit at startup instead of reading package.json per
  request on /version endpoints (all services)
- Standardize commit hash to 7 chars via .slice(0, 7) so CI full SHA
  and local short SHA produce consistent output
- Add try/catch for Explorer startup package.json read
- Remove redundant readFile calls from Keymaster and mediator startup
- Fix Swagger path annotations from /version to /api/v1/version
  (Gatekeeper, Keymaster)
