Ban git remote set-url SSH-to-HTTPS downgrade

Prevent Claude Code from silently changing a remote URL from SSH
(git@host:owner/repo) to its HTTPS equivalent, which breaks SSH key
authentication. The rule compares normalized canonical paths so it
only blocks same-repo protocol downgrades.
