Issue/fix relative url resolution (#16)

* Fix relative URL resolution to use document directory instead of file path

check_url_valid() joined relative URLs against document.path (a file
path), producing incorrect paths like /docs/report.md/foo.md. Changed
to document.path.parent so relative links resolve against the
document's directory. Added unit tests covering correct resolution,
file not found, and missing document path. Updated linter spec.

* Finalize issue with peer review findings

* Add relative URL prefix handling issue and update test coverage audit with known gaps
