Fix emergency plan form not scrollable when content exceeds screen (#587)

Replace the non-scrollable Column root with a single LazyColumn so the
title, form toggle, expanded form inputs, and step list all scroll
together. Previously the form card could grow beyond the viewport when
editing or adding actions, leaving the user unable to reach the save
button or steps list below.
