Close input forms by default on Sleep and Diary screens (#586)

SleepTrackingScreen and DiaryScreen had their forms visible by default,
inconsistent with all other screens (FoodLog, SportLog, Medication, etc.)
which default to collapsed. Set formVisible initial state to false on
both screens and update corresponding tests.
