Add per-skill concurrency protection to install/uninstall (#3851)

* Add per-skill concurrency protection to install/uninstall

Serialize Install and Uninstall operations on the same skill using a
per-skill mutex keyed by scope/name/projectRoot. This prevents races
when two concurrent REST API requests operate on the same skill (e.g.
two installs racing on extraction and store writes, or a simultaneous
install and uninstall).

Different skills proceed concurrently without blocking each other.

Closes #3845

* Canonicalize projectRoot before lock key and DB operations

Apply filepath.Clean to projectRoot in Install and Uninstall so
that equivalent paths (trailing slash, ".." segments) produce the
same lock key and database record.
