refactor: convert create/update to subcommand groups (create record, update record)

Renames the flat `ingitdb create` and `ingitdb update` commands to
`ingitdb create record` and `ingitdb update record`, matching the style
of `ingitdb delete record`. Exported functions remain Create/Update
(group wrappers); internal implementations use createRecord/updateRecord.

Also adds the countries collection as a standalone root collection for
manual CRUD testing.
