fix(e2e): prevent E2E tests from killing dev servers during self-development (#130)

Add defense-in-depth protection against accidentally killing dev servers:

1. Lock file mechanism: `make self` and `make run` create tmp/.dev-server-running
   containing the port number when a dev server starts.

2. Playwright config safety check: Before tests start, check for lock file.
   If found without PLAYWRIGHT_BASE_URL, fail with helpful instructions.

3. Convenience commands: Add `make self-test` and `make run-test` that properly
   set PLAYWRIGHT_BASE_URL to reuse existing dev servers.

4. Update CLAUDE.md: Replace scary "NEVER DO THIS" warnings with positive
   instructions showing the correct commands for each scenario.
