fix(test): use :memory: for truly isolated in-memory databases

The named in-memory database approach (file:UUID?mode=memory&cache=private)
was not providing proper isolation on Linux CI, causing test data to leak
between tests. Using the standard :memory: syntax creates completely
anonymous, isolated databases that cannot be shared between connections.
