Fix location_link output helper capture in violations form

Use Phlex `capture` to get location_link HTML as a string instead of
letting it write directly to the buffer. Without this, the location
violation highlight span was empty because the registered output helper
returns nil after writing to the buffer.

Add test assertion verifying location violation highlighting works.
