Commit Message

Fix permissions form checkbox layout and merge form title markup

- Remove form-control class from checkboxes (not meant for checkboxes in BS3)
- Override BS3 absolute-positioned checkbox layout in permissions table
- Add table class, vertical-align, and scoped styles for centered checkboxes
- Remove t() wrapper from description_title to avoid paragraph tag wrapping
