feat(monitor): extract freshness tracking into reusable module with escalation (#926)

Extract staleness detection logic from allocations.rs into a dedicated
freshness.rs module with FreshnessTracker struct.

Changes:
- Add FreshnessTracker with best_timestamp and last_fresh_update tracking
- Add FreshnessResult enum (Fresh, StaleButImprovement, StaleRejected)
- Implement escalation: warn → error after 2 hours without fresh data
- Include time_since_fresh in escalated log messages for visibility
- Comprehensive test coverage for all freshness scenarios

The tracker is designed for periodic queries where:
- Fresh data is always accepted
- Stale-but-improvement data is accepted (preserves progress)
- Stale-and-worse data is rejected (preserves last-known-good state)
- First fetch is always accepted (lenient on init)

This protects against Gateway routing to stale indexers while providing
clear operational visibility through escalating log levels.
