[+] more improvements to `index overview` dashboard  (#1154)

* Make `Top $n indexes by reads/hits` in terms of `bytes/second`

* refactor `reco_add_index` recommendations ordering

Order by `sum(avg_exec_time)` for all affected queries
instead of the biggest `total_exec_time` for any of the
affectted queries

Also return the affected query ids in case user wants to
investiage further

* Update `Missing Indexes` panel

* aggregate `pg_stat_statements` in `reco_add_index`
