Add V8 coverage collection fixtures for Playwright tests

Implements proper coverage collection for E2E tests:
- Add fixtures.ts with extended test fixture that wraps page
  with V8 coverage start/stop for monocart-reporter
- Update smoke tests to import from fixtures
- Add coverage output directory to gitignore
