[+] improve `Index overview` dashboard (#1146)

* Add `Top` variable to `index overview` dashboard

* Add `top n indexes by reads (blocks)` panel

* Add `top n indexes by cache hits (blocks)` panel

* Refactor `top n by cache-hits/reads` queries in `index overview` dashboard

* Add `idx_blks_read` and `idx_blks_hit` to the select of `index_stats`

* Add `top n indexes by number of tuples (read|fetched)` panels

* Update `Top n biggest indexes` to return number of scans in the specified interval

instead of the total number of scans previously

* Reorganize panels

* Revert removing `Source Name`

* Optimize query for `Top n biggest indexes` panel

* use `limit 100` in `index_stats` metric

As no panel queries more than 100 rows from it

* Rename panel

* Add panel descriptions
