refactor(reader): extract helpers from hasKeyInRange to reduce complexity

Extract prefix comparison logic into focused helper functions:
- isPrefixNotAfter: check if prefix <= key
- couldHavePrefixMatch: check if minKey could have prefix matches

Reduces cognitive complexity from 25 to below threshold.
