fix(web): use controlled inputs in product selection form

When migrating from plain HTML inputs to PF/Radio inputs, the HTML
checked attribute was not migrated to the corresponding isChecked prop.
This caused React warnings about components changing from uncontrolled
to controlled.

This change ensures these radio inputs are fully controlled.
