perf: append optimisations (#100)

* Add test to check records accumulate well

* Keep case-of simple

* Optimise message encoding

Try to take advantage of more tail-recursive, append-only binary
mutations, and local functions.

Highly depending on the input type, these optimisations make encode/1
~5-15% faster, and encode/2 ~0-20%.

* Apply a few more savings

* Minor parameter reordering
