refactor(test): implement file-level testing architecture (#203)

Reorganize test suite for better separation of concerns and maintainability:

- Move base class tests to test/unit/classes/ with focused fixtures
- Move utility tests to test/unit/utils/
- Centralize conversion correctness testing in integration/languages.test.js
- Simplify browser/UMD/CommonJS tests to focus on their specific concerns
- Add .markdownlint-cli2.mjs for auto-discovery of markdown files
- Document test responsibilities in test/README.md

BREAKING CHANGE: Test file locations changed (unit tests now in subdirectories)
