feat: Implement .rtmx/ directory structure and fix UnboundLocalError bugs

REQ-TEST-006: Fix UnboundLocalError bugs
- Add return statements after sys.exit(1) in 8 CLI files
- Prevents variable-before-assignment when sys.exit is mocked in tests
- Unskip and verify 5 previously failing tests

REQ-DX-001: Implement .rtmx/ directory structure
- Config discovery now checks .rtmx/config.yaml first
- Database discovery now checks .rtmx/database.csv first
- rtmx init creates .rtmx/ structure by default
- Add --legacy flag for old docs/ structure
- Create .rtmx/.gitignore to ignore cache/
- Add 12 new tests in tests/test_dx.py

Also adds Phase 4 requirement specifications:
- REQ-TEST-006 through REQ-TEST-009
- REQ-DX-001 through REQ-DX-006
