refactor(audit): extract audit package from server

- Move audit functionality to app/server/audit/ package
- Split into audit.go (middleware, interfaces) and handler.go (query API)
- Rename types to avoid stuttering: Store, Auth, Handler, Middleware
- Use *store.Store concrete type in server.go (similar to auth.Service)
- Consumer-side interfaces: audit.Store, web.AuditStore
