refactor: add mcp_version support and improve test formatting (#3)

TL;DR
Refactor formatter specs and add MCP protocol version support to
transport configuration.

## Changes

- Updated Ruby version from 3.4.7 to 3.4.8 in documentation and
configuration
- Added `mcp_version` field to `Entities::Transport` for MCP protocol
version configuration
- Enhanced `MCPExt::ServerFactory` to validate MCP protocol versions and
use the configured version
- Refactored formatter specs for better organization and readability
- Removed deprecated `spec/cases/node/render_markdown_spec.rb`
- Updated various test files to use improved formatting and structure
- Fixed minor formatting issues in README and documentation
