Phase 5: Create Bootstrap 4 mapping layer

Creates a comprehensive mapping between MO's standardized theme variables
and Bootstrap 4's variable system.

New Files:
- app/assets/stylesheets/mo/_map_theme_to_bootstrap4.scss
  Complete mapping of 121 theme variables to Bootstrap 4 variables

- doc/BOOTSTRAP4_THEME_MAPPING.md
  Comprehensive documentation of all mappings, unmapped variables,
  and migration strategy

- test/theme-validation/test_bootstrap4_mapping.scss
  Validation test file to verify mapping compilation

Features:
- Maps all 8 semantic colors (PRIMARY, SECONDARY, SUCCESS, etc.)
- Maps typography variables (body color, background, links)
- Maps component variables (buttons, forms, navigation, tables, etc.)
- Creates Bootstrap 4 $theme-colors map for automatic variant generation
- Thoroughly commented with 17 logical sections
- Zero hardcoded values - all from theme variables
