feat: Add SessionStart hook to install GitHub CLI on web sessions

Adds install-gh-cli.sh hook that downloads and installs the gh CLI
for Claude Code web sessions. This enables GitHub operations (PRs,
issues, etc.) in proxy-restricted web environments.

Runs on both startup and resume to ensure gh is available.
