Complete World.Has<T>(entity) with stale handle detection and comprehensive tests

Fix Has<T> to check IsAlive(entity) first, ensuring stale/destroyed entity
handles correctly return false instead of potentially returning true for
entities with matching IDs. Add comprehensive XML documentation and 21 unit
tests covering all acceptance criteria from issue #57.
