kubernetes: simplify IP address type resolution with clear precedence

- Prefer `ingress.kubernetes.io/ip-address-type`, fallback to deprecated `alb.ingress.kubernetes.io/ip-address-type`, else use flag default
- Warn on deprecated annotation usage
- Error when both annotations are present with different values to avoid ambiguity
- Update README with deprecation notice and precedence

All tests pass locally.
