#89: Update documentation for v0.4.0 release

  Comprehensive documentation update covering all v0.4.0 features and breaking changes.

New documentation:
- SQL Migrations guide with ISqlDB interface examples
- v0.3 to v0.4 migration guide with automated script
- Reorganized API interfaces into focused subdirectory

Breaking changes documented:
- API renames: migrate()→up(), migrateTo()→up(version), downTo()→down(version)
- Config: filePattern→filePatterns (array), checkConnection() required
- Interface/service method renames across all APIs

Updates:
- All code examples in guides, recipes, and configuration docs
- Fixed license.md structure and configuration syntax errors
