fix: respect existing env vars when loading .env (#239) (#249)

Changed load_dotenv/0 to only set environment variables from .env
if they are not already set in the system environment. This fixes
the issue where .env values would overwrite explicitly set env vars.

Before: PORT=1111 bin/myapp start would use PORT=9999 from .env
After: PORT=1111 bin/myapp start correctly uses PORT=1111

Fixes #239
