feat: unify reference and expected output directories (#51)

* feat: unify reference and expected output directories

- **Unify Reference Outputs**: Merge cudd_reference_outputs and teddy_reference_outputs into single reference_output directory
  - All 13 common files were identical between methods
  - Preserved 4 TeDDy-only edge case files
  - Updated all CMake test registrations to use unified directory
  - Updated PowerShell scripts for reference file generation

- **Unify Expected Outputs**: Merge cudd_expected_output and teddy_expected_output into single expected_output directory
  - All 78 common files were identical between methods
  - Preserved 2 TeDDy-only test files
  - Updated all CMake test infrastructure paths
  - Updated Python integration test paths

- **Update Documentation**: Comprehensive documentation updates
  - Updated all README, CONTRIBUTING, and doc files
  - Updated image paths in LESSONS_LEARNED and VISUALIZATIONS
  - Updated Doxyfile example paths
  - Updated troubleshooting and setup instructions

- **Infrastructure Changes**:
  - Modified cmake/test_functions.cmake for unified directory support
  - Modified cmake/run_bdd_test.cmake for unified reference handling
  - Modified cmake/python_bdd_integration.cmake for new paths
  - Updated scripts/update-reference-files.ps1 to output to unified directories
  - Fixed mermaid test registration to only include files with references

- **Benefits Achieved**:
  - Eliminated 91 duplicate files (78 expected + 13 reference)
  - Simplified maintenance with single directories
  - Reduced project complexity while maintaining full functionality
  - All 30 mermaid tests still pass (100% success rate)
  - All basic functionality tests still pass

This change provides significant maintenance benefits by eliminating unnecessary
duplication while preserving complete functionality for both TeDDy and CUDD methods.
