fix: Fix header row padding alignment in table layout

- Remove inconsistent negative margins (-mt-px -mr-px) that caused misalignment
- Replace pb-px with consistent py-2 padding for all table cells
- Use pl-0 pr-3 for checkbox cell and pl-3 pr-0 for delete button cell
- Use px-0 for input cell and add gap-0 to grid to prevent extra spacing
- Simplify grid column structure by removing wrapper divs with margins
- This aligns header rows with proper vertical and horizontal spacing

Fixes alignment issue in Request headers section where padding was inconsistent.

All 413 tests passing.
