Use CONCURRENTLY for appeal index migrations

Updated up and down SQL migration scripts to use CREATE/DROP INDEX CONCURRENTLY for idx_appeals_lower_created_by_status_lower_account_id. This allows index operations without locking the appeals table, improving migration safety and minimizing downtime.
