Centralize lcov configuration and version handling

- Centralize all lcov settings in src/Makefile with version-specific flags
- Add support for lcov v2 ignore-errors flags automatically
- Export LCOV and GENHTML variables for inheritance by sub-makes
- Remove duplicate lcov configuration from tests/Makefile
- Update run_test script to use inherited LCOV variable
- Simplify all lcov invocations to use centralized variables
- Ensure consistent lcov behavior across all coverage operations

This eliminates duplicate lcov version detection code and provides
a single source of truth for lcov configuration, making the build
system more maintainable and ensuring compatibility with both
lcov v1 and v2.
