fix: prevent double apple spawning in same frame

- Add frame-based spawn tracking to prevent multiple apple spawns per frame
- Replace Date.now() IDs with counter-based IDs to avoid collisions
- Add periodic cleanup of destroyed objects to prevent memory leaks
- Reset counters on game state reset

Resolves race condition between timer cleanup and collision detection
both triggering apple spawning in the same frame
