fix: minor web UI improvements from review feedback

- Set HttpOnly: true for theme cookie (no client-side JS reads it)
- Standardize Content-Type headers to always include charset=utf-8
- Remove redundant HX-Refresh header in view mode toggle (base template handles it via custom event)
