More expressive handling of format field liftovers

- Squashed merge of #91 and #92
- Format field default behavior: missing (fill with .) or zero (fill with 0); the latter is applicable for AD, former is generally applicable to most fields
- Option to ignore_non_variants (records) when populating format fields, ie to preferentially only use format field values from variant records, falling back to non-variant records only when no variant records are found for sample.
- Updated test cases and gvcf integrated tests
- Updated CLI config to apply default_type: zero to AD (default_type: missing applied by default to all other fields); ignore_non_variants: true for GQ.
