Construct refactoring (#82)

* Adds new utility constructs for common TLS protocol entities.

* Switch to PrefixedBytes where an explicit length field is not expected.

* Convert CipherSuites to TLSPrefixedArray.

* pep8 fixes

* Addresses code review comments from @ashfall and cleans up tls.test.test_utils.

Adds docstrings for the new constructs in tls.utils and the
corresponding tests in tls.test.test_utils.  Renames
tls.tests.test_utils.IntEnum to IntegerEnum, eliminating a naming
clash with enum.IntEnum.
