Distance PID improvement.

Previously it was possible to have the vision or range finder not on target,
but return a value so small that the distance PID loop would return
true from a call to onTarget().
This patch makes this impossible, by:
a) Checking that the vision/range is not already on target when calculating distances,
b) Making sure that vision/range are reporting as onTarget at values larger than
the threshold for the distance PID.
