rapidapp.pl - drop "upgrade" mode, add --dir option

The rapidapp.pl bootstrapper no longer supports the "upgrade" mode that
catalyst.pl does, since this is entangled with the much more complex
helper traits, and this style of updating apps has never been a
supported workflow.

Now that this mode has been excluded, cleanup code has been added which
cleans up created files/dirs if the process errors, making it safe to
run over and over w/o having to manually delete partially created files,
which is very confusing to users.

Additionally, the user can suppy the target dir if they choose with the
new --dir option. If not supplied, the standard dist-name style is used
like before.
