Consolidate/generalize around 'default_favicon_url'

The existing RapidApp config setting 'default_favicon_url' is now used
actively to set the favicon in html/templates generated by RapidApp. It
is now included in the (also existing) template statement

 [% c.all_html_head_tags %]

Which is already added to RapidApp's built-in html templates.

The favicon <link> tag can also be accessed from user-created templates
with:

 [% c.favicon_head_tag %]

It can also still be accessed via [% c.default_favicon_url %] -- the new
[% c.favicon_head_tag %] is just a fully constructed <link> tag vs just
the url.

Additionally, this setting is now included (commented-out) in newly
bootstrapped apps since this is a question that frequently comes up.
