Make Javadoc reassignment optional.

Default attribute assignment now behaves exactly as expected. In
particular, special handling of the `order` attribute is now removed.
Javadoc also by default gets attributed to the type that directly
follows it, except when the new `reassignJavadoc` option is set. (Since
Javadoc are separate from normal annotations, this makes reasoning about
annotations simpler in all cases: annotations are now _always_ assigned
to whatever directly follows.)
