Allow missing comments (#10)

* Better handle missing keys

invert length handling config so that missing keys don't cause an infinite loop

* test index type urls as well

* cleanup up the error messages

* change most logs to debug log to quiet output unless asked for

* add some tests to JS that this is handled as well

* small fixes
