Write the error message in the body if it's not used for headers

The response is processed as follow:

 - 'Content-Type' is set to 'text/plain; charset=utf-8'
 - 'Content-Length' is set to the size of the message, overriding the
   default chunked 'Transfer-Encoding'
 - the message is written in the response with 'write_all' to ensure
   it's written completely

Update the documentation to specify that the error message can also be
written in the body.

Add a test to cover the response object when such an error occurs.
