Add Cookies.lookup to find a cookie by its name

The implementations is based on Cookies.from_request_headers to find the
cookies matching the provided name.

 - the lookup is case-insensitive
 - null is returned if nothing is found

Improve the cookies documentation and add a section about the
'Cookies.lookup' function.
