Resources/images: update application icon (#110)

After morphing into our degree planner application, the address book 
application icon is no longer representative of what our application 
offers.

Let's update the application icon to make it more relatable and:
* rename `address_book_32.png` to `plan_with_ease_32.png`
* cascade the change of application icon filename in the codebase
* acknowledge the original source of the icon (Font Awesome) in `README`
* update `copyrights.txt` to conform to Font Awesome Free License[1] 
  (CC BY 4.0 License[2])
