Commit Message

Increase test code coverage.

Lots of filling of gaps and some refactoring. Lots more to do.

- Add coverage & build badges to both master & develop branches.
- Remove unit tests from code coverage. Slipped in at some point.
- Based on code coverage, eliminate dead code in NSURLRequest.
- First unit tests for session manager for streams & sessions.
- Backfill a previous race condition bug around queuing stream to one pool but it
  opening in another.
