Commit Message

Increase code coverage and fill in gaps.

Largely guided by code coverage reports. Still more to do.

Coverage said the read accessors on NSMutableURLRequest were never
used, so removed them.

Some refactoring here to support testability.

Added badge to README to track develop as well as master, and show both
build and coverage results.
